Ghana defender, Baba Abdul Rahman, is set to leave Chelsea after eight years following an agreement to terminate his contract with 12 months left.
The 29-year-old has reportedly agreed a deal to join Greek giants POAK in the summer transfer window.
Although Rahman was one of the players retained by Chelsea following the end of his loan spell at Reading, the defender wants to leave the club on a permanent deal.
According to transfer expert, Fabrizio Romano, Rahman has agreed to mutually terminate his contract so he could leave Chelsea as a free agent.
A two-year deal has been agreed between the player and POAK ahead of the official announcement of the move.
Rahman joined Chelsea in 2015 from German Bundesliga side Augsburg but struggled to break into the first team forcing him to spend most of his time on loan.
In 2021, Rahman spent a season with POAK, helping them win the Greek Cup, as reported by Transfermarkt.
He also played for Schalke in Germany, Real Mallorca in Spain as well as Stade Reims in the French Ligue 1. Rahman spent the last two seasons at English Championship side Reading FC.
Chelsea retain Baba Rahman
Earlier, Sports Brief reported that Baba Rahman's Chelsea future has been handed a huge boost after the Ghanaian defender was retained by the club ahead of the 2023/24 season.
The Black Stars left-back has a year left on his current deal with the Blues and will be expected to join the London club this summer for pre-season.

Ghana defender Baba Rahman has shared his experience at the World Cup in Qatar, after making a return to his club Reading FC.
The Black Stars left-back immediately joined his teammates for mid-season training ahead of the resumption of the English Championship.
Rahman played all three games for the Black Stars, as the four-time African champions exited the tournament at the group stage.
