The Italian, in his first major final, needed to recover from two sets down to beat Medvedev in sets of 3-6 3-6 6-4 6-4 6-3.
The win followed another outstanding performance against Novak Djokovic in the semifinals. It was another bitter experience for Medvedev after he also blew away a two-set lead to Rafael Nadal in the 2022 final. The Russian has now lost five of his six major finals.
In the women's final a day earlier, favourite Aryna Sabalenka saw off Zheng Qinwen to successfully defend her title. The Belarusian won the game at Melbourne Park in straight sets of 6-3 6-2.
