She was born on the 7th of January 1991 in Ga-masehlong, Limpopo, SouthAfrica. She was raised in Fairlie, Limpopo, by her grandmother. Her entire family, including her parents, Jacob and Dorcus Semenya, have come out strongly to defend Mokgadi in the past. Speaking to the Star, Dorcus said,

"I know who and what my child is. Mokgadi Caster is a girl, and no one can change that."

While her father, Jacob, told the Sowetan newspaper,

"She is my little girl. I raised her, and I have never doubted her gender. She is a woman, and I can repeat that a million times."

The International Amateur Athletic Federation (IAAF) categorizes and identifies her as a DSD or intersex athlete or an athlete with Differences in Sex Development. Although her testosterone levels are naturally higher than the average woman's, she has not come out to confirm whether she is an intersex woman or not.

All national athletic bodies are expected to test their athletes to ensure their testosterone levels are within the acceptable limits. Unfortunately, this ruling by the IAAF has led to the disqualification of countless athletes, especially if they decide not to suppress their testosterone levels.

After her gold win in the World Championships in 2009, the IAAF subjected her to a gender verification test and a ban from participating in any competitions. Nevertheless, the 2012 Sportswoman of the Year maintained a strong spirit during the 11 months of humiliating public scrutiny.

In 2016, she became the first athlete to win in the 400m, 800m and 1500m events in record time. Throughout the legal battle to compete fairly, she made astonishing improvements that the world began referring to as her 'acts of defiance.'

In 2016, South Africa's golden girl won her second Olympic gold medal alongside Margaret Wambui and Niyonsaba Francine. However, the famous trio, categorized as DSD athletes, were subjected to harsh criticism. Adoption of the 2019 IAAF rule meant that the trio had become ineligible to run in the 400m - 1500m races unless they took testosterone suppression for 6 months before racing.
